add some asserts in SwFormatAnchor

Which flushes out some dodgy behaviour.

Particularly in SwUndoFormatAttr, which was storing a sal_Int32 content offset in a sal_uInt16 page field.
In this case, rather just store the value on SwUndoFormatAttr itself, the same way it does for other things.
