tdf#42949 Fix IWYU warnings in sfx2/source/[s-v]*/*cxx and sfx2/qa
Also quickly recheck other parts of sfx2 after recent welding there
Found with bin/find-unneeded-includes
Only removal proposals are dealt with here.
