A temporary composed list of file types and extension types is build
from the original list and provided to the IFileSaveDialog object,
so that this dialog shows only one extension.

These are all the cases where the warning suggests a direct replacement.
This is a companion to 2f2c9df8e270ede0fd71b146380c3883b75d8f0d
"-Werror,-Wdeprecated-declarations (--with-macosx-version-min-required=10.14)"
which covered all the other cases (via SAL_WNODEPRECATED_PUSH/POP) that offered
no direct replacement.

* expanding STDAPI to its definition (as per
<https://msdn.microsoft.com/library/ms686631(vs.85).aspx> "STDAPI"), to add
__declspec(dllexport) into its middle, in
extensions/source/activex/so_activex.cxx; as discussed in the comments at
<https://gerrit.libreoffice.org/#/c/60691/> "Get rid of Windows .def files in
setup_native, use __declspec(dllexport)", having a function both listed in a
.def file EXPORTS and marking it dllexport is OK, and the latter helps the
heuristics of loplugin:external; however, the relevant functions in
extensions/source/activex/so_activex.cxx probably don't even need to be
exported in the first place?

The problems with moving entities into unnamed namespacs and breaking ADL
(as alluded to in comments in compilerplugins/clang/external.cxx) are
illustrated by the fact that while

namespace N {
struct S2: S1 { int f() { return 1; } };
int f(S2 s) { return s.f(); }
}

returns 1, both moving just the struct S2 into an nunnamed namespace,

While the folder picker had some shared code with the file picker,
the folder hierarchy made some sense, but since it's gone now,
move the files to the base directory.

Citing SHBrowseForFolderW remarks: "For Windows Vista or later, it
is recommended that you use IFileDialog with the FOS_PICKFOLDERS
option rather than the SHBrowseForFolder function."
Since the minimum required Windows version is already 7 and we
already just provide the Vista file picker, this gets rid of the
Windows XP compatible folder picker code.

in files formerly omitted for oversight or non-cxx extension

Since LO message processing loop is a little more complex then
PeekMessageW and DispatchMessageW, use Reschedule( true ) to
process all pending events. Processing all event is important,
because MsgWaitForMultipleObjects will just trigger for new
messages since its call and will ignore already pending
messages.
Which leaves the IsMTA() call. I know of the COM STA / MTA
stuff, but I don't understand how this might be related here.
The call to SHBrowseForFolderW definitly block, but we already
use multiple threads, so I don't understand, how this has
worked before...

It didn't help putting a SolarMutexGuard into
VCLXButton::ProcessWindowEvent, which was my first suspect in my
own backtrace, as VCLXWindowImpl::OnProcessCallbacks releases it
to process the callbacks.
Since the gtk backend also guards all calls into the file picker
code, I think it's save to do the same for the Windows picker.
I have no idea where it is actually released again...
