And make it simple to disable the whole feature by setting
gb_DISABLE_PCH_REUSE=1, just in case.
Also work around a possible BOOST_ALL_NO_LIB mismatch when
using the common PCH.

Where reasonable means they are from a list of defines known not
to affect the system headers, and so they are safe to differ from
how the PCH was built. A bit hackish, but works in practice.

Using SAX2 interface provides element/attribute names split into prefix/name,
and provides namespaces/urls, so FastSaxParserImpl::callbackStartElement()
does not have to figure out these on its own (and additionally libxml2
spends a noticeable portion in xmlStrdup() when not in SAX2 mode).
The switch saves about 5-10% time.

expat_utf16 is long gone, so there is no need to fool around with
expat_utf8.

Naming convention for gbuild methods:
- "add" is used for stuff that is logically a part of the target
(i.e. not registered at the Module, but defined in the target's makefile)
- "use" is used for stuff that is logically a different target
(i.e. it is registered at the Module, has it's own makefile, may be
in a different module than the target)
