So that we have fewer cases to deal with when we transition to
32-bit bitmaps.
(*) rename maMask to maAlphaMask, since now it is only being used
for alpha duties.
(*) drop mbAlpha and mbTransparent to simplify state management,
the only thing we need to check for alpha is if maAlphaMask is
non-empty.

Since "forever", SalAbort has been declared as non-exported in
vcl/inc/salinst.hxx and (only) called from Application::Abort
(vcl/source/app/svapp.cxx) in Library_vcl. Its various implementations for
different platforms have always been scattered across Library_vcl.
For Windows, SalAbort was originally implemented in vcl/win/app/salinst.cxx,
until 1698debed2993fc5f262aa3ebbdb32fc112ac556 "Implement Windows VCL backend as
plugin" introduced an incompatible implementation in vcl/win/app/salplug.cxx
(which was added to Library_vcl) and moved the original implementation in
vcl/win/app/salinst.cxx from Library_vcl to Library_vclplug_win, where it thus
became dead code (and where it now gets removed).
For macOS, SalAbort was originally implemented in vcl/osx/salinst.cxx, until
3af4e1a0825c5b11ae4ef58fc411378aab669387 "Implement MacOSX VCL backend as
plugin" introduced a different implementation in vcl/osx/salplug.cxx (which was
added to Library_vcl) and moved the original implementation in
vcl/osx/salinst.cxx from Library_vcl to Library_vclplug_win, where it thus
became dead code (and where it now gets removed).
(In 0f3be2e19fa408d7069d586ccf04cb3f3eccd6b9 "Unify sal plugin loaders", the---
identical---new implementations in vcl/osx/salinst.cxx and
vcl/win/app/salinst.cxx where then consolidated with other---also identical---
implementations in vcl/source/app/salplug.cxx.)
For macOS, the original, now removed implementation in vcl/osx/salinst.cxx and
the consolidated implementation in vcl/source/app/salplug.cxx only differed in
an added
CrashReporter::addKeyValue("AbortMessage", rErrorText, CrashReporter::Write);
which is presumably harmless to add.
But for Windows, the original, now removed implementation in
vcl/win/app/salinst.cxx differed substantially from the consolidated
implementation in vcl/source/app/salplug.cxx, which is updated here to reflect
those differences. The one thing that cannot easily be updated, though, is the
//TODO: ImplFreeSalGDI();
call, as ImplFreeSalGDI is defined in Library_vclplug_win. I'll thus leave
fixing that TODO for another commit (if calling ImplFreeSalGDI from SalAbort is
even necessary, given that it ends in FatalAppExitW anyway)---my gut feeling is
that the whole 1698debed2993fc5f262aa3ebbdb32fc112ac556 "Implement Windows VCL
backend as plugin" was somewhat misguided.

For some unclear reason, for me at least, when I have built with Xcode
11.1 or 11.2 and run on macOS Cataline, I run into a crash caused by
boundless recursion related to the progress bar in the recovery
dialog. Not setting mbProgressNeedsErase to true for the "NWF" seems
to help.
Whether this has some unintended visually bad side effect I don't
know, but hey, it's just a progress bar, who cares if it in some
circumstances doesn't look perfect?

This restores the nested NSApplicationMain and default run loop
usage. Without it the Java AWT integration will start its own
event loop, effectively blocking any non-system event processing.
Reproducible via "Tools - Macros - Organize Macros - BeanShell...
- LibreOffice Macros - HelloWorld - helloworld.bsh - Edit".
The blocking can be prevented by overriding NSApplication::run and
running our own event loop using Application::Execute. But this
still doesn't show the Java AWT editor window and I couldn't find
any information how to fix this.
Since OSX now is a VCL plugin, this can't restore the old hook
mechanism, but instead adds a new function to SalInstance.
SalInstance initialization happens at InitVCL() start just a
little bit later in the call stack.
Somehow NSApplicationMain manages to run the Java VM in an extra
thread, so it doesn't block the main loop. Probably this could
also be handled by LO starting the JVM as a thread.
Further information for an implementation eventually can be found
in the "Technical Note TN2147" "JNI Development on Mac OS X."

The graphicsPort property of NSGraphicsContext is deprecated in 10.14.
CGContext is there since 10.10 so we can use it unconditionally.
But sadly this did not enable me to un-comment the assert() a few
lines below.
Wonder if this deprecation is just a cleanup of the API, and
functionality-wise the CGContext property is exactly the same as
graphicsPort?

This reverts c3c88d6bbe4109fcf1a6d53e947faec775c96e42 "loplugin:redundantcast"
(whose commit message was rather misleading; the redundantcast warnings were
merely a follow-up to the commit's actual meat, of replacing deprecated uses of
'graphicsPort' with 'CGContext'; I had failed to realize that when I chunked the
various changes necessary for 10.14 into several individual commits) and adds
SAL_WNODEPRECATED_DECLARATIONS_PUSH/POP instead.

(aka 'CGContext *') prvalue is redundant" with
--with-macosx-version-min-required=10.14 at least. (Not exactly sure why this
warning only starts to appear now. Presumably something changed in the system
headers. Lets see whether older baseline builds are happy with the dropped
casts, too.)

Allows to drop all the special SVMain handling introduced for
MacOSX. This way LO can also be build via SSH, because gengal
won't abort / fail because of the missing window system.
Currently this just implements the global menu. I don't know
what else is missing. Eventually we want to reimplement the
Cocoa debug option forwarding (NSAccessibilityDebugLogLevel).

After the refectoring in commit 4c93de2c921b ("merge
GenericSolarMutex and SolarMutex"), there is no more need to
prevent instantiation of comphelper::SolarMutex objects.
Since every VCL backend implements the yield mutex management in
the same way, we can move the general implementation into the
SalInstance.
While at it use std::unique_ptr for the yield mutex on Mac and
Windows platforms.

rtl/string.hxx and rtl/ustring.hxx both unnecessarily #include <sal/log.hxx> (and don't make use of it themselves), but many other files happen to depend on it.
This is a continuation of commit 6ff2d84ade299cb3d14d4110e4cf1a4b8070c030 to be able to remove those unneeded includes.
This commit adds missing headers to every file found by:
grep -FwL sal/log.hxx $(git grep -Elw 'SAL_INFO|SAL_INFO_IF|SAL_WARN|SAL_WARN_IF|SAL_DETAIL_LOG_STREAM|SAL_WHERE|SAL_STREAM|SAL_DEBUG')

On some Macs, it seems that LibreOffice (or any app?) gets an
NSApplicationDidChangeScreenParametersNotification as soon as it has
started and asked for such a notification. Our handler for that
notification assumes that VCL is initialised. Thus we should not ask
for such notifications before VCL has been initialised.
I could not reproduce the reported crash with an unmodified
LibreOffice, only after inserting a sleep after the notifications had
been set up. But I am fairly sure this change fixes the problem.
